Financial PlanningWe will be happy to discuss the Surgery Center fees and the billing process with you prior to receiving services. Please bring your current insurance cards and picture identification with you the day of the surgery The Surgery Center fee covers the use of the facility. The patient is responsible for payment of deductibles and co-insurance payments prior to surgery. Services performed by laboratory, pathology, your surgeon, or anesthesiology, are billed separately. The following is accepted in lieu for cash payment: Medicare, Medicaid, Worker's Compensation (verified and authorized), Group Insurance (verified and authorized), and Contracted Insurance (verified and authorized). We accept Visa and Mastercard and Discover. |

Medicare Patients have an annual deductible and a 20% co-payment for which they are responsible. If you have a Medi-Gap plan, it may cover your deductible. you will need to verify this with your insurance company.
Workers Compensation patients need to provide the following information: Employer's Name, benefit administrator (insurance company), industrial claim number, and date of injury or illness. This will be verified and authorized prior to surgery. If the procedure is not authorized, guidelines for private pay patients will apply.
Medicaid patients are not responsible for any balances due after Medicaid has paid for services. In order to be considered a Medicaid patient, you must present, at time of registration, a Medicaid Card that is valid for the month in which you are receiving services. If you do not have a current card, guidelines for private pay patients will apply.
Group Insurance Patients have an annual deductible and co-payment for services. The balance of the deductible and the co-payment is the patient's responsibility prior to surgery.
Private pay patients (no insurance) will be required to pay a 50% deposit and sign a promissory note for the balance prior to surgery. Please contact the Business Manager to make payment arrangements as soon as your surgery has been scheduled.
